What is bipolar disorder?

Bipolar disorder is a mood disorder marked by episodes of depression and episodes of abnormally elevated or irritable mood called mania or hypomania. Between episodes, many people feel well. Without treatment, though, mood swings can disrupt careers, finances, and relationships.

Signs of mania or hypomania include decreased need for sleep, racing thoughts, rapid speech, unusual confidence, risky spending or behavior, and irritability.

How Dr. Green can help

Bipolar disorder is frequently misdiagnosed as depression, which can lead to treatments that don't work or that make mood swings worse. Dr. Green, a board-certified psychiatrist with more than 40 years of medical experience, takes a careful history of past mood episodes, sleep patterns, family history, and substance use to reach an accurate bipolar diagnosis. For teens, he also distinguishes bipolar disorder from ADHD, depression, and normal adolescent moodiness.

What treatment looks like

Bipolar disorder is a long-term condition, and good treatment is a long-term partnership. Care focuses on stabilizing mood, preventing future episodes, and protecting the things that matter in your life. Regular follow-up visits track mood, sleep, medication effects, and early warning signs. Psychotherapy helps you understand your patterns and triggers, rebuild relationships affected by past episodes, and stay engaged with treatment. Steady sleep and routines are a key part of the plan.

Is medication involved?

Yes. Medication is the foundation of bipolar disorder treatment. Mood stabilizers such as lithium, lamotrigine, and valproate, along with certain atypical antipsychotics, help prevent manic and depressive episodes. Some of these medications require periodic blood work to check levels and monitor kidney, thyroid, or liver function. Dr. Green explains each option, balances effectiveness against side effects, and adjusts treatment carefully over time. Antidepressants are used cautiously, because on their own they can trigger mania.

Common questions about bipolar disorder

Is bipolar disorder the same as mood swings?

No. Everyone has ups and downs. Bipolar disorder involves distinct episodes lasting days to weeks, with clear changes in sleep, energy, thinking, and behavior that are noticeable to others.

Can bipolar disorder be treated without medication?

Psychotherapy, routine, and good sleep are important, but for most people with bipolar disorder, medication is essential to prevent relapse. Stopping mood stabilizers suddenly raises the risk of a new episode, so changes should always be made with your psychiatrist.
